Starting July 1, 2026, the California Department of Health Care Services (DHCS) is making changes to Medi-Cal dental coverage (Denti-Cal) that will affect some adults but not all. If you or a family member rely on Denti-Cal, here is a plain-language explanation of who is affected, who keeps full coverage, and the practical steps to take before the deadline.

What is changing on July 1, 2026?

California is restricting full-scope Denti-Cal dental benefits for some adult enrollees based on immigration status. After July 1, 2026, adults age 19 and older who do not qualify for federal full-scope Medi-Cal will only receive emergency dental services through Medi-Cal, that is, treatment for severe pain, infection, or urgent dental problems. Routine cleanings, regular fillings, crowns, and most other non-emergency services will no longer be covered for this group.

Who is affected by this change?

The change applies to adults age 19 and older whose Medi-Cal eligibility is based on state-only (not federal) funding. In practice this primarily affects undocumented adults who currently have full-scope Medi-Cal benefits. If this describes your situation, beginning July 1, 2026 your Denti-Cal benefits will be limited to emergency dental services only.

Who keeps full Denti-Cal coverage?

Several groups keep full dental coverage regardless of immigration status: children and teens under 19, adults who are pregnant or within one year after the end of pregnancy, adults who qualify for federal full-scope Medi-Cal (typically based on lawful presence and other factors), and residents of skilled nursing or long-term care facilities. If you or a family member fall into one of these groups, no action is required.

When will notification letters arrive?

DHCS started mailing notices to affected members in April 2026. The notice explains how your benefits will change and how to continue receiving emergency dental care after July 1. What should you do before July 1?

Emergency dental care will still be covered for everyone, that includes severe tooth pain, dental abscess, infection, traumatic injury, and other urgent problems.
